Your 45-day free home trial will be honored for 45 days, starting from the date we ship your custom earmold. The custom earmold becomes yours at the moment we ship it. Both parties agree that the sale occurs in Minnesota and that Minnesota law shall govern the making and enforcing of this purchase agreement. Kustom Ear Gear fully complies with all governmental regulations as stipulated by Minnesota state law, FDA, FTC, and the laws of interstate commerce. 

Waiver to Medical Evaluation Requirements: 

I have been advised by Kustom Ear Gear that the Food and Drug Administration and the State of Minnesota have determined that my best interest would be served if I had a medical evaluation by a licensed physician, preferably a physician who specializes in diseases of the ear, before purchasing a hearing instrument; or a test by a licensed hearing aid professional utilizing established procedures and instrumentation in the fitting of hearing instruments. I am 18 years of age or older and voluntarily accept this waiver which indicates that I do not wish a medical evaluation or test before purchasing a hearing instrument. I agree to pay for merchandise ordered or to return it in original condition at the end of the 45 day trial period.
